The Problem with a Hyperactive Amygdala
In people with anxiety disorders, this alarm system becomes miscalibrated. The amygdala fires in response to perceived threats — a critical email, a social situation, an uncertain future — with the same intensity it would reserve for a physical predator. The body doesn't know the difference.
Chronic activation of the stress response has measurable effects on the brain. The hippocampus — responsible for memory and context — shrinks under prolonged cortisol exposure. The prefrontal cortex, which normally regulates the amygdala's responses, becomes less effective at doing its job. The result is a feedback loop: anxiety makes the brain more anxious.
What Happens During a Panic Attack
A panic attack is the amygdala's alarm system firing at maximum intensity. Heart rate surges, breathing becomes shallow, muscles tense, and the body prepares for immediate physical action. The terrifying part is that these sensations — racing heart, dizziness, chest tightness — are then interpreted as evidence of danger, which amplifies the alarm further.
Key insight: Panic attacks are not dangerous. They are intensely uncomfortable, but the physical symptoms are the result of your nervous system doing exactly what it was designed to do. No one has ever died from a panic attack itself.
How CBT Rewires the Anxious Brain
Cognitive Behavioural Therapy works by targeting the thought patterns that feed the anxiety cycle. Through a process called cognitive restructuring, patients learn to identify and challenge catastrophic thinking — the mental habit of assuming the worst-case scenario.

- Behavioural experiments — testing feared predictions in real life
- Exposure therapy — gradual, systematic confrontation of feared situations

Neuroimaging studies have shown that CBT produces measurable changes in brain activity. After successful treatment, the amygdala shows reduced reactivity to threat cues, and the prefrontal cortex shows increased activity — evidence that therapy literally changes the brain.
The Role of Avoidance
One of the most important — and counterintuitive — findings in anxiety research is that avoidance maintains and worsens anxiety. When you avoid a feared situation, you get short-term relief, but you also send a message to your brain: "That thing was genuinely dangerous." The amygdala's threat response is reinforced, not extinguished.
This is why exposure therapy — deliberately approaching feared situations — is so effective. It allows the brain to learn, through direct experience, that the feared outcome does not occur. This process, called extinction learning, gradually reduces the amygdala's threat response.
