How Trauma Rewires the Brain — And How EMDR Helps Undo It

Traumatic memories are not stored like ordinary memories. When a traumatic event occurs, the brain's normal memory consolidation process is disrupted. Instead of being processed and integrated into the autobiographical narrative, traumatic memories are stored in a raw, fragmented form — vivid, sensory, and disconnected from time.
The Neuroscience of Trauma
During a traumatic event, the amygdala — the brain's threat detector — goes into overdrive. The prefrontal cortex, which normally provides context and rational appraisal, is effectively taken offline. The hippocampus, which encodes memories with time and context, is impaired by the flood of stress hormones.
The result is a memory that lacks the contextual markers that tell the brain "this happened in the past." When triggered, the traumatic memory is re-experienced as if it is happening now — hence flashbacks, nightmares, and the hypervigilance characteristic of PTSD.
Key concept: PTSD is not a sign of weakness. It is a normal nervous system response to an abnormal event. The brain is doing exactly what it was designed to do — keeping you alert to danger. The problem is that it cannot turn the alarm off.
What Is EMDR?
Eye Movement Desensitisation and Reprocessing (EMDR) is a structured therapy that uses bilateral stimulation — typically guided eye movements — while the patient holds a traumatic memory in mind. Developed by Francine Shapiro in the late 1980s, EMDR is now endorsed by the WHO, the American Psychological Association, and the UK's NICE guidelines for PTSD.
How EMDR Works
The exact mechanism of EMDR is still debated, but the leading theory is that bilateral stimulation mimics the eye movements of REM sleep — the phase during which the brain processes and consolidates memories. By activating this process while the traumatic memory is held in working memory, EMDR allows the brain to reprocess the memory, integrating it into the autobiographical narrative and reducing its emotional charge.

- History taking and treatment planning
- Preparation — building coping resources
- Assessment — identifying target memories
- Desensitisation — bilateral stimulation while processing the memory
- Installation — strengthening positive beliefs
- Body scan — addressing residual physical tension
